From my understanding, the laboratory information system can be paper based or electronic one. So, when you're having an electronic LIS, you can state in your contingency plan to use the paper-based LIS as the alternative to be used in case of electronic LIS failure/interruption.

I got the same idea with Mr. Musa. Some form that store in LIS should be available in the lab for using when the system is down. The reserve form should be formatted constantly with the form printed from LIS and all the process should be started in SOP. The back up forms, SOP, and also NCs for LIS downtime are the evidences.

Yes agree with having a paper based system but always include it in the periodic review. As this is one of the things we overlook on our laboratory.
I do agree with previous comments on utilizing a paper-base system as a back-up for your on-site LIS in the event of system failure, fire etc. Consider the following as a part of the contingency plans:

We have manual forms as a backup in the event we encounter problems related to our LIS. It is also very important to have this enrolled and reviewed regularly to make sure the forms are aligned with the current methodologies and other formats the laboratory is using.
