The greatest challenge I faced as laboratory manager in the implementation of occurrence management program was getting staff to report occurrences. Most of the reports that were recorded in the incidence logbook were those that I reported myself. How do you get your staff to report occurrences in your lab?

Build confidence amongst Lab staff and stop use of threats. Provide some incentives and simplify the CAPA / NCE / NC Form for reporting Occurrences.
