Root cause analysis (RCA) is a framework for structured investigations of safety incidents. Undertaking root cause analysis remains a complex non-linear task which entails balancing a multiplicity of concerns and expectations. Root cause analysis is not just for safety incidents but all every incidents. The major challenge in my facility most of the new staff do not understand the systematic approach to RCA irrespective of the method used in the conduct of the RCA
