We get to the root cause by performing root cause analysis using many approaches. These include five whys, fishbone, brainstorming, pareto chart. No for every occurrence to happen, there must be a root cause.
Yes. in situation where the occurrence is addressed fully with remedial action. Usually for minor occurrences.

I am not good at RCA but I just share my way, use the ISHIKAWA principle like: 4M+1E.
But you can do it as your suitable way. The objective just to prevent re-occurred and to detect the trend of bigger problem in the future.

I share Daron suggestion , in addition it is obvious NC could not happen without root cause , but some NC (minor) either fix by immediate action or no need of RCA due to the low risk empact . Plus we are not spend our precious time by investigating the RCA for every occurred NC , but prioritize using Risk analysis as filter .
